Payment for Service:
Payment is due at the time services are rendered. For your convenience, we do accept VISA, MasterCard, Discover, American Express and CareCredit.

                

Insurance:
Payment for services is the responsibility of each patient. If you have insurance coverage, we request that you pay your co-payment and deductible at each appointment. Even though your insurance benefits may cover a larger portion of the cost, you will be responsible for any part not covered by your insurance at the time of treatment.

Tuesday, Wednesday, Thursday 8 a.m. - 5 p.m.
Friday 8 a.m. - 1 p.m.
